As in all medical practices there are certain forms needed prior to your first visit with us. To try to make this process easier and more efficient we are providing these forms here. You can print these forms, fill them out, and bring them with you or they will be provided upon arrival. If you have questions filling out the forms feel free to call or email us for assistance.

- Medical Problems and Complaints Form
  (necessary for each new medical problem)

- Summary of Notice of Privacy Practices Form

- Financial Policy Form

In addition to the forms above those patients that have MediCare as their primary insurance will also need to complete these forms:
   - Medicare Authorization Form
   - Medicare Patients Nail Policy Form
